Pricing: $300 per applicant

This will cover the cost of the medical examination and any required lab work. This fee does not include any additional vaccinations you may be required to receive for I-693 completion.

Before your appointment, please bring:

• Identification (ex: Driver’s License, Passport)
• Vaccination Record, if any
• Medical Record, if any

At your appointment, you can expect:

• Medical examination
• Review of vaccination records
• We will instruct you if you require further vaccinations.
• Tuberculosis Screening: TB QuantiFERON Gold blood test. 
• If your test result is positive for TB antigens, you will be required to receive a chest x-ray.
• Syphilis Screening: Rapid Plasma Reagin (RPR) blood test.
• Gonorrhea Screening: Urine Nucleic Acid Amplification (NAA) Test

After your appointment:

Your results will be documented on USCIS Form I-693 and placed in a sealed and signed envelope. The envelope must not be opened by the applicant. Give the sealed envelope to your immigration lawyer or mail it directly to USCIS. You will also receive a scanned copy of the completed I-693 form and lab test results in your email.

Should you have positive findings on any required lab test results or a health condition that requires further medical treatment, clearance will not be granted until all necessary follow-up appointments are completed.

How long will my I-693 Immigration Physical be valid for?

Forms signed and completed on or after November 1, 2023, will not expire and can be used indefinitely, per USCIS.

Do you accept health insurance for the medical exam?

No, health insurance is not accepted or required for the immigration medical exam. The fee is $300 per applicant and can be paid with cash or credit card.

Do I need to fast before my appointment?

No. Please eat and drink water before your appointment.

Do I need to complete the necessary lab testing before my appointment?

No. All required lab work will be completed at Yi Wellness during your appointment.

Do I need to receive all required vaccinations before my appointment?

No. Our staff will let you know which vaccinations you must receive to complete the I-693 form.

When can I expect my I-693 form to be completed?

I-693 forms are typically completed within 10-14 business days.

Can I receive my required vaccinations at Yi Wellness?

Unfortunately, we do not provide vaccinations for applicants in our office at this time.

My vaccination records are from another country and/or are not in English. Will you still accept them?

Yes. Any official written vaccination record from any country in any language is acceptable. Please bring them in for your appointment, and our staff will assist you in determining if any further vaccinations are required.

What should I do if I only have a digital copy (photo, scan, MyChart, etc.) of my vaccination record?

Please have the digital copy of your vaccination records on hand when you arrive for your appointment. A physical copy of your vaccination record is not required.

Is the COVID-19 vaccine mandatory?

No, the COVID-19 vaccine is no longer mandatory as of January 22, 2025, per USCIS.
